Immunization Requirements

Below are the MANDATORY immunization and health related requirements according to state law KRS 241.034

REQUIRED IMMUNIZATIONS FOR KINDERGARTEN ENTRY

902 KAR 2:060 Requires the following immunizations for all children at least five (5) years of age:

1. Five (5) doses of DTaP or DTP or combinations of the two vaccines.

2. Four (4) doses of IPV or OPV or combinations of the two vaccines.

3. Three (3) doses of Hepatitis B

4. Two (2) doses of MMR; and

5. Two (2) doses of varicella, unless a healthcare provider states that the child has had a diagnosis of typical varicella disease or verification of a history of varicella disease by a healthcare provider or a diagnosis of herpes zoster disease or verification of history of herpes zoster disease by a healthcare provider.

6. Two (2) doses of Hepatitis A

PHYSICAL EXAM

704 KAR 4:020 requires each child first entering a Kentucky public school to present a Kentucky Preventative Health Care Examination Form. This exam may be completed up to one year prior to initial entry.

VISION EXAM

KRS 156.160 (1) (h) requires proof of a vision examination by an optometrist or ophthalmologist. This evidence shall be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5) or six (6) year old is enrolled in public school, public preschool, or Head Start program.

DENTAL SCREENING 

KRS 156.160 (1) (i) requires proof of a dental screening or examination by a dentist, dental hygienist, physician, registered nurse with training, nurse practitioner, or physician assistant. This evidence shall be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old is enrolled in public school.  ***A valid exam/screening must be completed in the year the child enrolls 1/1/23-12/31/23. Exams completed in preschool/Head Start do not meet this requirement unless they were performed during 2023.***

THE KENTUCKY IMMUNIZATION CERTIFICATE AND THE PREVENTATIVE HEALTH CARE EXAM FORM ARE REQUIRED TO BE ON FILE PRIOR TO THE 1ST DAY OF ENTRY. PARENTS ARE URGED TO MAINTAIN COPIES OF ALL MEDICAL DOCUMENTS.

Below are the MANDATORY immunization and health related requirements that must be met PRIOR TO THE 1ST DAY OF SIXTH GRADE ENTRY according to state law KRS 241.034

IMMUNIZATION REQUIREMENTS FOR SIXTH GRADE

For sixth grade entry, age eleven (11) or twelve (12) years or older a child shall have received:

(a) One (1) dose of Tdap if it has been at least two (2) years since the administration of the last dose of tetanus-containing vaccine

(b) Two (2) doses of varicella, unless a healthcare provider states that the child has had a diagnosis of typical varicella disease or verification of a history of varicella disease by a healthcare provider or a diagnosis or herpes zoster disease or verification of a history of herpes zoster disease by a healthcare provider;

(c) One (1) dose of MCV or MPSV (meningococcal vaccine)

(d) Two (2) doses of Hepatitis A 

Your physician should issue a new, complete Kentucky Immunization Certificate (updated version) with all vaccines recorded.

PHYSICAL EXAM REQUIREMENTS

Kentucky Preventative Health Care Examination Form is required for all students entering 6th grade.  This exam may be completed up to one (1) year prior to entry into 6th grade.  

ATHLETIC PHYSICALS

A separate sports/athletic physical completed on a KHSAA form is required annually to participate in strenuous sports at the middle or high school level.  The KHSAA sports physical DOES NOT replace the required Kentucky Preventative Health Care Exam required for sixth grade entry.

In order for your student to complete enrollment into middle school, the KY Immunization Certificate and KY Preventative Health Care Exam Form must be on file at your child’s school prior to the first day of attendance.  Parents are urged to maintain copies of all medical documents.

OUT-OF-STATE ENROLLEES

Out-of-State enrollees may be required to provide their out-of state immunization documents to the Oldham County Health Department for transfer PRIOR to enrollment. Depending on your child’s current immunization documents, we may be able to accept the certificate as is.

If a Kentucky certificate is necessary, the Oldham County Health Department will provide the parent/guardian with an Immunization Transfer Receipt which is then given to the school in which the student is enrolling.
