Enrollment Procedures

Enrollment in a school is done by the parent/guardian after it has been determined which school the child will attend. (Click here for district map and school locator.)  For additional assistance in learning which school a child will attend, please contact the Department of Pupil Personnel at 241-3500.

School registration for the coming year is held during the second semester of each school year and during the summer, prior to the opening of school. Parents who miss the registration dates and parents relocating within the District or from outside the District are urged to register their children at their school as soon as possible before the first day of classes.

In order to enroll the child, the parent and child go to the school the child will attend and bring with them the following items:

PROOF OF RESIDENCY 

Proof of residency in Oldham County is required. Parent/Guardian should bring a utility bill (LG&E, KU, Louisville Water or Oldham County Water) or a rental agreement, if applicable. A printed version of your online utility bill is acceptable.

You may also be asked to provide a Residency Affidavit Form. This may be necessary if you cannot provide the documentation listed above.

Driver’s licenses are not acceptable to prove residency. 

BIRTH CERTIFICATE 

Kentucky law requires a certified copy of the student’s birth certificate upon enrollment. The school must view the original copy received from the state and make a photocopy for the student’s file. Hospital and billfold size birth certificates are not acceptable.

SOCIAL SECURITY CARD

Please provide your child’s social security card to the school. A photocopy will be made and placed in your child’s file.

SCHOOL RECORDS

The parent/guardian should bring the name and address of the person to whom the receiving school may write to obtain the student's school records directly. Also, the parent should bring a recent report card, achievement test results, class schedule, or other information to be used in appropriately placing the student until records arrive.

CUSTODY - LEGAL DOCUMENTS (if applicable)  

It is the parent/guardian responsibility to keep the school informed of changes in custody by providing the office with current and complete legal documents each year. These legal documents must be on file in the school office prior to the first day of school.

IMMUNIZATION CERTIFICATE

A current immunization certificate is required to be on file at all times in order to attend KY schools.

(KRS 158.035) each student must have a current Kentucky Immunization Certificate on file in the school office at ALL times. The school will keep the original form.

  • In-State enrollees will be required to provide proof of a current Kentucky Immunization Certificate upon enrollment
  • Out-of-State enrollees: are required to provide their immunization document to the Oldham County Health Department for transfer PRIOR to enrollment. The Oldham County Health Department will provide the parent/guardian with an Immunization Transfer Receipt which is then provided to the school to complete enrollment.

KENTUCKY PREVENTATIVE PHYSICAL EXAMINATION

A Kentucky Preventative Physical Examination is required for all students.

(704 KAR 4:020) A Kentucky Preventative Health Care Examination is required for ALL enrolling students. This physical examination must be completed on a Kentucky Preventative Health Care Examination Form.

This examination will be required UPON ENROLLMENT for a child’s initial entry into Kentucky public school (K- Grade 12) and/or when entering into sixth (6th) grade within a period of twelve (12) months prior to the first day of school. This form must include a medical history, growth and development, general appearance, physical assessment, and recommendations to the school regarding health problems that may require special attention in the classroom or physical education activities. The school will keep the original form. Out-of-state enrollees have thirty (30) to complete this requirement.

ANNUAL SPORTS PHYSICAL (if applicable)

Required annually for students participating in middle/high school athletics. The KHSAA sports physical exam does not replace the required “Kentucky Preventative Physical Exam” required for sixth grade entry. Contact your school’s Athletic Director regarding requirements for each school level.

EYE EXAMINATION

Any student under the age of seven (7) entering a public school for the first time must have an eye examination by an optometrist or ophthalmologist.  This will need to be done before January 1.

(KRS 156.160(h).)  Proof of a vision examination by an optometrist or ophthalmologist completed on Kentucky Eye Examination Form for School Entry.  This evidence shall be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5), or six (6) year old child is enrolled in public school, public preschool, or Head Start Program. The school will keep the original form.

DENTAL EXAMINATION

Any five (5) or six (6) year old entering a public school for the first time must have a dental examination by a dentist, dental hygienist, physician, Health Department registered nurse with KIDS Smiles Training, advanced registered nurse practitioner or physician assistant. This will need to be done before January 1.

(KRS 156.160(i).)  Requires proof of a dental screening or examination by a dentist, dental hygienist, physician, health department registered nurse with KIDS Smiles Training, advanced registered nurse practitioner, or physician assistant completed on the Kentucky Dental Screening/Examination Form for School Entry.  This evidence shall be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old is enrolled in public school.

FREE AND REDUCED PRICE SCHOOL MEALS INFORMATION

Children from families whose income is at or below certain established levels are eligible for free or reduced-price meals. Families may apply for this benefit at any time during the school year. All decisions on free and reduced-price meals are made according to annually updated federal guidelines. For more information, call 241-3500.
